Commitment to HIPAA Compliance

SLP Fusion, a wholly owned subsidiary of Aerodigestive Health Corporation (“AHC”), is committed to protecting the privacy and security of personal health information in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its implementing regulations.

This HIPAA Statement describes how SLP Fusion handles Protected Health Information (“PHI”) that may be collected, used, or disclosed through our website and services.

What is Protected Health Information (PHI)?

Protected Health Information, or PHI, refers to individually identifiable health information that is transmitted or maintained in any form or medium, whether electronic, paper, or oral, and relates to:

  • The past, present, or future physical or mental health or condition of an individual,

  • The provision of health care to an individual, or

  • The past, present, or future payment for the provision of health care to an individual.

How We Protect PHI

SLP Fusion implements administrative, physical, and technical safeguards designed to protect PHI from unauthorized access, use, or disclosure. These measures include, but are not limited to:

  • Secure, encrypted transmission and storage of PHI,

  • Access controls and authentication procedures,

  • Regular staff training on HIPAA compliance,

  • Policies and procedures for handling and disposing of PHI.

Use and Disclosure of PHI

SLP Fusion will only use or disclose PHI as permitted or required by law, including:

  • For treatment, payment, or health care operations,

  • With your written authorization,

  • As required by law, regulation, or court order.

We do not sell or rent PHI to third parties. Any third-party service providers that may have access to PHI are required to sign Business Associate Agreements (BAAs) and adhere to HIPAA requirements.

Your Rights Under HIPAA

If SLP Fusion collects or maintains your PHI, you have the right to:

  • Access and obtain a copy of your PHI,

  • Request corrections to your PHI,

  • Request restrictions on certain uses or disclosures,

  • Request an accounting of disclosures,

  • File a complaint if you believe your privacy rights have been violated.

Notice to Users

Not all information collected through the SLP Fusion website is considered PHI under HIPAA. For example, information collected for website analytics, marketing, or general inquiries may not be protected by HIPAA, but will be handled in accordance with our Privacy Policy.
